Job Description:
A company is looking for a Senior Living Care Navigator.
Key Responsibilities:
Conduct telephonic outreach to members and assist with care
coordination
Guide members on utilizing plan benefits and complete telephonic
assessments
Facilitate communication between members and the care team,
providing community resource information
Required Qualifications:
High School Diploma / GED or equivalent work experience
Must be 18 years of age or older
2+ years of experience in a healthcare setting with patient and
medical team communication
Experience working in cross-functional, inter and
multi-disciplinary teams
Proficiency with EMRs and knowledge of medical terminology
