Job Description Job Summary:This position is responsible for assessing, reporting, and coordinating care for clients in line with the overall clinical component of the program. Day-to-day responsibilities may vary.Job Duties and Responsibilities:Core duties and responsibilities include but are not limited to the following. Other duties may be assigned as necessary. Assess client current conditions and health problems and/or needs. Assess client for drug intoxication and withdrawal. Assess clients for medication side effects, including adverse drug reactions. Assess barriers to learning. Demonstrate the ability to establish rapport with clients, families, and other caregivers. Assess clients throughout their stay to determine the clients’ response to care and when there are significant changes in the clients’ condition. Ability to identify clients’ level of honesty and self-motivation. Demonstrate the ability to assess for abuse and neglect. Document assessments in accordance with established policies and procedures. Demonstrate knowledge of detoxification protocol and medications used in the detoxification process. Carry out care for client safely and effectively in accordance with the clients’ physician. Insure all medications are locked safely and retrieved for self-administration by clients. Supervise self-administered medications with documentation as directed. Coordinate nutritional screenings on all clients to determine clients’ nutritional needs and to identify clients at risk. Provide safe and effective care for clients identified to be at risk of injury to self or others. Interact with clients in accordance with the goals and interventions outlined in their respective treatment plans and specific needs. Demonstrate the ability to interact appropriately with clients at all times. Demonstrate the ability to therapeutically confront clients and their family members as indicated. Communicate effectively with outside physicians and other appropriate parties in order to plan, deliver, and evaluate care. Ensure client compliance with facility rules and guidelines. Complete documentation in the client chart in a thorough, timely, legible, and accurate manner. Report incidents of suspected/known abuse/neglect to the Executive Director and complies with all mandatory reporting laws. Facilitate the destruction of medication. Communicate with MD regarding client medication orders and client symptoms. Provide daily nursing services such as quality nursing care, individual counseling, medication support, educational programs, and case management services for clients. Participate in updating of treatment plans, and monitor interactions, side effects, etc., in consultation with Executive Administrator, physicians, psychiatrists, and all other treatment staff. Chart and report any significant changes in condition and general progress of residents to Executive Director, Clinical Director, physicians and psychiatrists. Monitor all medications and sign and certify the accuracy of counts and inventory each day. Follow and adhere to all medication policies regarding dispensing, administration, record keeping, storage and accountability for inventory. Participate in staff meetings, team meetings and in-service trainings. Maintain ethical boundaries with clients. Special Projects as needed.Required Education and Experience:• Minimum of 2 years of clinical experience preferred.• Required current certification as an RN, LVN, or LPN preferred.• Preferred 2 years of experience in medical facility, mental health or chemical dependency setting; Work and/or life experience accepted if approved by Administration.• If recovering, at least 3 years of sobriety is preferred.• Valid Drivers License.Required Knowledge, Skills, and Abilities:• Knowledge of substance abuse related issues and the recovery process.• Knowledge of the recovery field including the recovery practices and the 12-step process.• Ability to maintain ethical and professional boundaries with clients and staff members.• Ability to summarize pertinent clinical information via written correspondence.• Ability to prioritize task demands within established time frames.• Knowledge of the process and signs of acute withdrawal from substances, detoxification protocol and related care.• Skilled in case management.• Time management and crisis intervention• Proficiency in Microsoft Office Applications including Excel, PowerPoint, and Word.• Knowledge of HIPAA regulations and guidelines.• Must be organized, detail oriented, and have the ability to multitask.• Must be flexible in work hours in order to meet facility needs.• Excellent verbal and written communication skills.• Critical thinking and problem solving skills.• Must be able to work in a team environment.• CPR certification.
